Do I Need TRT? Low Testosterone Signs, Testing, and Treatment

How do you know if you need testosterone replacement therapy (TRT)? You need two things: symptoms of low testosterone, and blood work that confirms it. The American Urological Association uses a total testosterone level below 300 ng/dL, measured on at least two separate early morning blood tests, as a reasonable cutoff for low testosterone. Symptoms alone are not enough, because fatigue, low drive, and weight gain can have many causes. Below, the team behind the men's TRT program at Healthy Living Medical in Texarkana explains the signs, the testing, and what treatment actually looks like.

Common signs of low testosterone

  • Low energy and fatigue that sleep does not fix
  • Lower sex drive or erectile changes
  • Brain fog, poor focus, or low motivation
  • Loss of muscle mass and strength, even with regular workouts
  • Increased belly fat
  • Irritability, low mood, or feeling "flat"
  • Poor sleep or less frequent morning erections
  • Reduced bone density over time

Testosterone naturally declines about 1% per year after age 30 or 40, but it is not just about age. Obesity, type 2 diabetes, sleep apnea, certain medications, heavy alcohol use, chronic stress, and problems with the testes or pituitary gland can all lower it.

How is low testosterone diagnosed?

A proper diagnosis starts with lab work drawn in the morning, usually before 10 a.m., when testosterone is highest. Because levels bounce around day to day, guidelines recommend confirming a low result with a second test. A thorough panel often includes:

  • Total testosterone and often free testosterone
  • LH and FSH, which help show whether the issue starts in the testes or the brain
  • Estradiol
  • Complete blood count, especially hematocrit
  • PSA for prostate screening, based on age and risk
  • Other labs as needed, such as thyroid, prolactin, A1c, or lipids

What is TRT?

Testosterone replacement therapy restores testosterone to a healthy range in men who are confirmed to be low. It comes in several forms:

  • Injections, usually weekly or twice weekly, which are the most common and affordable option
  • Topical gels or creams, applied daily
  • Pellets placed under the skin every few months

Your provider will recommend a form and dose based on your labs, lifestyle, and preferences, then adjust it based on follow up testing.

How long does TRT take to work?

  • 3 to 6 weeks: many men notice better energy, mood, and libido
  • 2 to 3 months: improved focus and sleep, and often better erections
  • 3 to 6 months: changes in muscle, strength, and body fat become more visible
  • 6 to 12 months and beyond: improvements in bone density

Is TRT safe?

When it is prescribed for men with confirmed low testosterone and monitored properly, TRT has a solid safety record. The large TRAVERSE trial, published in 2023, found that testosterone therapy did not significantly increase major cardiovascular events like heart attack and stroke compared with placebo in men with low testosterone and heart risk factors. TRT still has risks that need watching:

  • Higher red blood cell count (hematocrit), which is checked with routine blood work
  • Lower sperm production and fertility. TRT signals your body to make less of its own testosterone and sperm. If you want children now or in the future, tell your provider before starting so you can discuss fertility friendly options.
  • Acne, fluid retention, breast tenderness, or changes in estradiol
  • Prostate monitoring with PSA according to your age and risk

This is why "TRT without labs" is a red flag. Ongoing monitoring is what keeps therapy safe.

Local TRT clinic vs online TRT

Searches for "online testosterone therapy" and "testosterone therapy clinics near me" have both grown in Texas this year. Online programs can be convenient, but you should know what you are giving up. A local clinic lets you sit down with a provider, get a hands on evaluation, draw labs on site or nearby, and see the same team for every adjustment. Whichever you choose, look for confirmed lab diagnosis before treatment, a clear monitoring schedule, and a provider you can actually reach.

Can lifestyle changes raise testosterone?

Sometimes, yes. Losing excess weight, treating sleep apnea, strength training, cutting back on alcohol, and improving sleep can raise testosterone in some men. For men carrying extra weight, our GLP-1 weight loss program may help on its own or alongside TRT. Your provider can help you decide whether lifestyle changes, treatment, or both make the most sense.

Frequently asked questions

What testosterone level is considered low?

The American Urological Association uses a total testosterone below 300 ng/dL on two separate morning tests, along with symptoms, as the guide for diagnosing low testosterone.

At what age should men get their testosterone checked?

There is no set age. If you have symptoms like persistent fatigue, low libido, or loss of muscle at any adult age, it is reasonable to get tested.

Is TRT a lifelong commitment?

For many men it is long term, because stopping usually means levels drop back down. Some men whose low testosterone is tied to weight or sleep issues may be able to come off therapy after addressing the cause.

Does TRT cause infertility?

It can lower sperm production. If fertility matters to you, talk with your provider before starting.

Does insurance cover TRT?

Coverage varies by plan. We will walk you through costs and options during your consultation.
